The Effect of Polydeoxyribonucleotide on Chronic Non-healing Wound of an Amputee: A Case Report

Polydeoxyribonucleotide (PDRN) is safe and effective in wound healing, cellular growth, synthesis of extracellular matrix protein, and inflammation reduction via activation of adenosine A2 receptors. We report a 28-year-old male patient treated with PDRN injections for chronic non-healing wound refractory to negative pressure wound therapy, skin graft, or growth factors. Three injections of PDRN were administered at the wound site into the anterior and medial sides of the left stump on the 1st, 4th, and 9th days of hospitalization. The PDRN ameliorated wound healing by enhancing cell growth, tissue repair, and angiogenesis. PDRN application represents a potential treatment for non-healing wounds obviating the need for additional therapies, and hospitalization, as well as improve patient’s activities of daily living.

Factors Affecting Wound Healing after Decannulation of Tracheostomy Tube.

Objective
To see the factors affecting wound recovery of the patients who decannulated tracheostomy tube and to provide an index to guide proper time for stomaplasty. Method: The subjects were 41 patients having received tracheostomy tube decannulation. The medical records of the subject patients were investigated retrospectively. Among the group with natural sealing off, the factors affecting spontaneous closure were compared in terms of the duration of tracheostomy tube cannulation, age, hemoglobin, protein, albumin, peak flow meter, mini-mental status examination (MMSE), and oral feeding. Results: The group with natural sealing off had 142 days as the average period of tracheostomy tube cannulation while the group without showed 652.33 days. The duration of tracheostomy was statistically significant in linear regression analysis. In addition, the groups with oral feeding and without oral feeding were significantly different from each other (p<0.05). Conclusion: Significant factors affecting wound recovery after tracheostomy tube decannulation were the duration from tracheostomy operation to tracheostomy tube decannulation and oral feeding. The Effect of Extracorporeal Shock Wave Therapy on Pressure Ulcer.

Objective
To investigate the effect of extracorporeal shock wave therapy (ESWT) on pressure ulcers which is a major, functionally-limiting medical problem impairing quality of life for many people each year. Method: Seven patients who had stage 3 pressure ulcers were enrolled for the study. Each patient was treated with 6 sessions of low-energy (0.10∼0.15 mJ/mm2, 1,000 impulses) ESWT. The length, width, depth and soft tissue biopsy of pressure ulcers were evaluated every 2 weeks for 6 weeks. Results: The length, width and depth decreased significantly after 2 weeks of ESWT application. Healthy granulation tissue was formed. Soft tissue biopsy revealed increased the number and size of capillaries and decreased inflammatory cells in treated case. Conclusion: ESWT promoted wound healing and revealed favorable histological changes in pressure ulcers. We suggest that ESWT can be used for the safety and effective management of pressure ulcer. Waveform Effects of Electrical Stimulation on Wound Healing in Rats.

Objective: To examine the effects of two stimulation waveforms on wound healing in rats.

Method: 30 Sprague-Dawley rats with a 7-mm diametrical round incision including skin and subcutaneous tissue on the dorsum were divided into three groups. A(n=10) and B groups(n=10) were given rectangular and triangular monophasic pulsed current 1 hr daily until the wound healed completely (duty cycle 10%, absolute spatial current density 0.022 mA/cm2 for A group and 0.011 mA/cm2 for B group), and C group (n=10) received no electrical stimulation. As the indicator of wound healing effect, percentage of reduction in wound area, duration of complete healing and antibacterial effect were evaluated and statistically compared among three groups.

Results: The mean values of percentage of reduction in wound area were 75.1⁑3.0% for A group, 74.2⁑5.1% for B group and 68.7⁑5.4% for C group. The mean values of duration of complete healing were 11.1⁑1.3 days for A group, 12.0⁑1.4 days for B group and 16.1⁑1.0 days for C group. The mean antibacterial effect were 0.5⁑0.5 for A group, 0.6⁑0.5 for B group and 2.1⁑0.6 for C group on 2 days post wounding, and 0.4⁑0.5 for A group, 0.5⁑0.5 for B group and 1.4⁑0.7 for C group on 6 days post wounding. Although those for A and B groups were significantly different from those for C group, no significant difference in all values of three indicators existed between A and B groups.

Conclusion: Results suggest that two stimulation waveforms have no significantly different effects on wound healing in rats.
