Unusual Pharyngo-Esophageal Dysphagia Caused by a High-Riding Right Brachiocephalic Artery

High-riding right brachiocephalic and subclavian arteries are often asymptomatic and rare vascular variations. We present a patient with high-riding right brachiocephalic and subclavian arteries that caused compressive pharyngeal and cervical esophageal dysphagia. An 82-year-old woman presented to our hospital with dysphagia that worsened with solid food. A pulsatile lump was observed on the right neck side. A videofluoroscopic swallowing study showed a deviated food bolus movement anterolaterally from the normal vertical pharyngoesophageal transition, with a filling defect in the lower pharynx through the upper esophagus. Neck computed tomography (CT) showed high-riding right brachiocephalic and subclavian arteries and a tortuous right common carotid artery located adjacent to the trachea in the cervical area. The cervical midline structures were deviated to the left neck side. A neck mass with vascular variation should be considered as a cause of dysphagia that worsens with solid food; CT should be considered to determine its cause.

Investigating the Dose-Related Effects of Video Game Trunk Control Training in Chronic Stroke Patients With Poor Sitting Balance

Objective
To investigate the dose-related effect of trunk control training (TCT) using Trunk Stability Rehabilitation Robot Balance Trainer (TSRRBT) in chronic stroke patients with poor sitting balance.
Methods
This was a retrospective study of 38 chronic stroke patients with poor sitting balance that underwent TCT with TSRRBT. The participants were assigned either to the low-dose training (LDT) group (n=18) or to the highdose training (HDT) group (n=20). In addition to the conventional rehabilitation therapy, the LDT group received 5 sessions of TSRRBT intervention per week, whereas the HDT group received 10 sessions of TSRRBT intervention per week. The outcome measures were the scores on the Trunk Impairment Scale (TIS) and its subscales, Berg Balance Scale (BBS), Functional Ambulation Classification (FAC), and the Korean version of Modified Barthel Index (K-MBI). All outcome measures were assessed before the training and at the end of the 4-week training.
Results
After the 4-week intervention, TIS, BBS, FAC, and K-MBI scores showed improvement in both LDT and HDT groups. Furthermore, the improvements in TIS scores and its subscales were significantly greater in the HDT group than in the LDT group (p<0.05).
Conclusion
TCT using TSRRBT could be an additional treatment for the conventional rehabilitation therapy of chronic stroke patients with poor sitting balance. HDT may provide more beneficial effects on improving patients’ sitting balance than LDT.

Case Report

The authors report the diffusion tensor tractography (DTT) findings of three pediatric patients with gait dysfunction and corticoreticular tract (CRT) disruption. All three patients showed unilateral trunk instability, but they did not show any spasticity or weakness of the distal extremities. Clinical evaluation of trunk instability using a Trunk Control Measurement Scale (TCMS) revealed that the more affected side had a lower score than the contralateral side. DTT showed disrupted CRTs in hemispheres contralateral to the hemiparetic sides, which were associated with unilateral proximal instability, although conventional MRI showed no abnormal lesion explaining the hemiplegic symptom. Compared to the results in age-matched controls, these three patients had decreased values of fractional anisotropy (FA) and tract volumes (TV) of the affected CRTs, and these values were also decreased compared to those in the contralateral side. On the other hand, values of FA and TV of the corticospinal tracts on the ipsilateral and contralateral sides were only marginally different. In conclusion, diffusion tensor imaging can be helpful for investigating the state of the CRT in pediatric patients with trunk instability and gait dysfunction.

Effects of Abdominal Hollowing During Stair Climbing on the Activations of Local Trunk Stabilizing Muscles: A Cross-Sectional Study

Objective

To examine using surface electromyography whether stair climbing with abdominal hollowing (AH) is better at facilitating local trunk muscle activity than stair climbing without AH.

Methods

Twenty healthy men with no history of low back pain participated in the study. Surface electrodes were attached to the multifidus (MF), lumbar erector spinae, thoracic erector spinae, transverse abdominus - internal oblique abdominals (TrA-IO), external oblique abdominals (EO), and the rectus abdominis. Amplitudes of electromyographic signals were measured during stair climbing. Study participants performed maximal voluntary contractions (MVC) for each muscle in various positions to normalize the surface electromyography data.

Results

AH during stair climbing resulted in significant increases in normalized MVCs in both MFs and TrA-IOs (p<0.05). Local trunk muscle/global trunk muscle ratios were higher during stair climbing with AH as compared with stair climbing without AH. Especially, right TrA-IO/EO and left TrA-IO/EO were significantly increased (p<0.05).

Conclusion

Stair climbing with AH activates local trunk stabilizing muscles better than stair climbing without AH. The findings suggest that AH during stair climbing contributes to trunk muscle activation and trunk stabilization.

The Effect of Chronic Low Back Pain on Bone Mineral Density and Trunk Muscle Strength in Women.

Objective: To evaluate (1) the effect of chronic low back pain on bone mineral density, and (2) the effect of chronic low back pain on trunk muscle strength in women.

Method: Subjects were 118 women patients (ages of 28∼65 years) suffering from chronic low back pain more than three months and control groups were 218 healthy women (age of 26∼72 years). We measured bone mineral density (BMD) at the lumbar spine and proximal femur using Dual X-ray absorptiometry and trunk muscle strength using Cybex 660 dynamometer.

Results: There was no significant difference between groups in lumbar spine BMD, in proximal femur BMD, or in trunk flexor muscle strength. However, trunk extensor muscle strength was reduced significantly in patient group. Trunk extensor muscle peak torque was 64.69⁑18.48 Nm in the patient group and 73.84⁑22.50 Nm in the control group in 30o/sec, and 58.65⁑18.59 Nm in the patient group and 65.68⁑20.28 Nm in the control group in 60o/sec.

Conclusion: The results suggest that chronic low back pain does not affect the BMD. However, it causes trunk extensor muscle weakness; therefore, trunk extension exercise will be help to the patients with chronic low back pain.

The Effects of Isokinetic Strengthening of Trunk Muscles on Balance in Hemiplegic Patients.

Objective
To identify the effects of isokinetic strengthening of trunk muscles on balance in hemiplegic patients after stroke. Method: All participants were ambulatory hemiplegic patients, injured at least 6 months prior to study. The patients (n=16) were randomly divided into two groups. The control group received neurodevelopmental treatments and gait training 3 times a week. In addition to the same treatments provided for the control group, the experimental group received trunk isokinetic strengthening exercises using isokinetic dynamometer 3 times a week. Trunk muscle peak torque and balance in the experimental group were compared with those in control group at baseline and 4 weeks after treatments. Results: There were no significant differences in age, lesion of hemiplegia, time after stroke onset, trunk muscle strength and Berg balance score before treatments between two groups. In the experimental group, peak torques of trunk extensor increased significantly at 60°/sec and 90°/sec at 4 weeks (p<0.05), but there were no significant improvements in peak torques of trunk flexor at all degree and extensor peak torques at 120°/sec. Both the extensor and flexors showed no significant changes in the control groups. In the experimental group, a mean percent change in peak torque involving the trunk extensor was significantly increased at 60°/sec. In the experimental group, Berg balance scores, 10 m gait velocity, sit to stand and gait 3 m and return, and 10 seconds stair up and down were improved (p<0.05). No significant improvements were noted in the control group. Conclusion: Isokinetic strengthening of trunk muscles can improve balance in hemiplegic patients. Trunk Impairment Scale for Evaluation of Functional Improvement in Acute Stroke Patients.

Objective
To evaluate the change of balance of the trunk using the Trunk Impairment Scale (TIS) in acute stroke patients and to compare it with the improvement in activities in their daily living (ADL) and gait. Method: The mean days from the onset of stroke were 11.3 (6∼17) days, and functional improvement of 24 hemiplegic patients was evaluated using the trunk impairment scale, Berg balance scale, timed up and go test, 6 minutes walk test, and the modified Barthel index every week for 4 weeks' conventional rehabilitation programs. Correlations between the trunk impairment scale and the other parameters were evaluated. Results: The trunk impairment scale and the modified Barthel index, both which showed significant improvement after 4 weeks, had statistically significant correlation (p< 0.05). The Berg balance scale had not improved significantly in 4 weeks. The timed up and go test and the 6 minutes' walk test could not be evaluated in most of these patients within 4 weeks. Conclusion: The trunk impairment scale could be a useful parameter for evaluating activities in their daily living (ADL) improvement in acute stroke patients who are unable to ambulate independently. And good trunk balance in acute stroke period is positive correlation with ambulation potential. (J Korean Acad Rehab Med 2010; 34: 278-284)

Trunk Muscle Strength and Bone Mineral Density in Women.

Objective: The purpose of this study was to investigate a correlation between the muscle strength of trunk and bone mineral density (BMD) in women.

Method: A total of 218 healthy women participated in the study. Their age ranged from 26 to 72 years. Dual X-ray absorptiometry was used to measure the BMD of lumbar spine and the trunk muscle strength was assessed by a Cybex NORMTM system. Under the standard criteria of World Health Organization for the dual X-ray absorptiometry analysis, we divided spine T-score into three groups(group 1: osteoporosis, group 2: osteopenia, and group 3: normal).

Results: The data revealed a siginificant correlation (r=⁣0.455, p=0.0001) between the age and BMD of lumbar spine. Trunk extensor muscle strength revealed 56.27⁑18.08 Nm (mean⁑SD) in osteoporosis group, 72.84⁑21.69 Nm in osteopenic group, and 77.90⁑22.28 Nm in normal group. Trunk flexor muscle strength was 82.73⁑23.30 Nm in osteoporosis group, 86.00⁑19.77 Nm in osteopenic group, and 98.91⁑18.29 Nm in normal group.

Conclusion: These results indicated that the trunk extensor muscle was weaker than the flexor muscle in osteoporotic group. As the bone mineral density reduced, the strength of both trunk extensor and flexor decreased. However the weakness of trunk flexor occurred at the earlier stage of osteoporosis than the weakness of trunk extensor.

Prediction of Functional Outcome after Stroke Using Acute Clinical Factors.

Objective
To plan the goals of the rehabilitation management after stroke, it was important to know functional prognosis of the patients. The purpose of this article was to predict functional outcomes of stroke patients by means of an analysis of the well-known prognostic factors of function at admission.

Method: This study was performed retrospectively on 102 patients with acute stroke who were admitted to Department of Rehabilitation Medicine. The inpatients, clinical and functional evaluation were carried out at admission. Functional abilities were measured with the use of the Functional Ambulatory Category (FAC) and the modified Barthel Index (MBI).

Results: The significant prognostic factors of FAC improvement rate were age, National Institute of Health Stroke Scale (NIHSS), Morticity Index, MBI, Mini-Mental State Examination (MMSE), aphasia and Trunk Control Test (TCT) (p<0.01). The most valuable single factor of FAC improvement rate was TCT. The significant prognostic factors of MBI improvement rate were age, NIHSS, Morticity Index, MMSE, aphasia and TCT (p<0.01). The most valuable single factor of MBI improvement rate was TCT.

Conclusion: We concluded that TCT could be the most valuable prognostic factor in rehabilitation management outcome of stroke.

Concentric and Eccentric Isokinetic Trunk Muscle Evaluation in Chronic Low Back Pain.

Objective: The purpose of this study is to evaluate concentric and eccentric trunk muscle strength in patients with chronic low back pain and compare it with healthy controls to determine the role of trunk muscle in chronic low back pain

Method: Subjects were 20 chronic low back pain patients with the duration at least 6 months and 20 normal adults. Concentric and eccentric isokinetic trunk muscle test was performed at specific speeds (30o, 60o, 120o/sec) with Cybex 770 machine in back pain patients and healthy control.

Results: The strength of trunk flexor and extensor muscles of chronic low back pain patients was significantly weaker than that of normal controls. Eccentric peak torque was significantly higher than concentric peak torque in both patients and healthy control. A concentric and eccentric ratio of patients was lower than that of normal controls. The correlation between concentric and eccentric variables was high. Our results show that concentric peak torque has the highest correlation with eccentric peak torque.

Conclusion: Isokinetic evaluations of the trunk muscle offer objective and quantitative data of patients with chronic low back pain and will be clinically useful in muscle power measurement of lumbar muscles.

Influence of Hemispatial Neglect on Trunk Control in Stroke Patients.

Objective
To evaluate the influence of hemispatial neglect on trunk balance control and functional ability in stroke patients. Method: Fourty eight inpatients exhibiting (n=25) or not exhibiting (n=23) hemispatial neglect following strokes within 6 months were matched for age and sex. Hemispatial neglect was assessed with a line bisection test (LBT) and a baking tray task test (BTT). Static and dynamic trunk balance control was assessed using a Balance Master System. Static balance control was measured with a weight bearing test and a Modified Clinical Sensory Interaction Balance Test (mCSIBT). Dynamic balance control was measured using a Limit of Stability (LOS) test and a rhythmic weight shift (RWS) test. A Functional Independence Measure (FIM) for functional ability and the Brunnstrom stage assessment for motor recovery were also recorded. Results: There was no statistical difference in the achievement of static trunk balance control between neglect and non-neglect patients. Where dynamic balance control was concerned, both LOS and RWS tests showed a deterioration in neglect patients compared to non-neglect patients (p<0.05). The Brunnstrom stage correlated significantly with dynamic balance control only in non-neglect patients (p<0.05) and the FIM score of neglect patients was significantly lower than that of non-neglect patients (p<0.05). Conclusion: Stroke patients with hemispatial neglect displayed pronounced dynamic trunk instability and functional dependency compared to patients without it. However, the recovery of dynamic balance control may not relate to the degree of motor recovery in stroke patients with hemispatial neglect. (J Korean Acad Rehab Med 2009; 33: 463-469)

The Effect of Neuromuscular Electrical Stimulation on Trunk Control in Hemiparetic Stroke Patients.

Objective
To determine the effectiveness of electrical stimulation over the trunk in improving trunk control during early stroke rehabilitation. Method: Thirty-two acute and subacute hemiparetic stroke patients were enrolled in this study. All patients were randomly assigned to one of two groups, control group or electrical stimulation (ES) group. Both groups received the same physical therapy for 3 weeks. ES group received additional electrical stimulation over the posterior back muscles for 30 minutes a day, 5 days per week for 3 weeks. Outcome measurements included Korean version of Berg balance scale (K-BBS), total score of postural assessment scale for stroke patients (PASS), trunk control subscale of postural assessment scale for stroke patients (PASS-TC), trunk control test (TCT), Korean version of modified Barthel index (K-MBI), and motricity index (MI). These measurements were checked before and after treatment. Results: There were no significant differences in the baseline characteristics and initial values between two groups. Following 3 weeks therapy, the changes of TCT and PASS-TC scores in the ES group were significantly higher than those of the control group (p<0.05). The changes of K-BBS, PASS, and K-MBI scores in the ES group tend to be higher than those in the control group. However the differences were not statistically significant. The changes of PASS-TC and TCT scores were significantly correlated with the changes of BBS and K-MBI scores at discharge. Conclusion: This study suggests that electrical stimulation over the trunk may be a beneficial therapeutic technique in improving trunk control in acute and subacute hemiparetic stroke patients.
