Case Report

Acute Retropharyngeal Calcific Tendinitis: Two Cases Reports.

Acute retropharyngeal calcific tendinitis is a rare benign condition which is characterized by acute neck pain and stiffness, and dysphagia. In this report, we present 2 cases of retropharyngeal calcific tendinitis patients. The first patient complained of neck pain and stiffness. The second patient had neck pain and hypophonia. In both cases, CT and MRI of the cervical spine demonstrated prevertebral effusion of the upper cervical spine with calcification at C1-C2 level. Acute neck pain and stiffness improved with oral administration of nonsteroidal anti-inflammatory drug. Follow-up radiologic findings revealed the disappearance of prevertebral effusion and calcification. (J Korean Acad Rehab Med 2009; 33: 718-722)

The Effect of Paratendinous Aprotinin Injection in Patients with Rotator Cuff Tendinitis.

Objective: To determine the therapeutic effect of paratendinous injection of aprotinin, a polyvalent inhibitor of inflammatory proteolytic enzyme, in patients with shoulder tendinitis. Method: Thirty patients with shoulder tendinitis diagnosed with ultrasonography were included. Patients were assigned to one of two groups at random to receive paratendinous injection. One group received a paratendinous aprotinin 1.5 ml and 1% lidocaine 2 ml injection of shoulder 2∼5 times at 1 week apart. The other group received a paratendinous injection one time with mixture of triamcinolone 40 mg and 1% lidocaine 2.5 ml. The effect of treatment was assessed with the visual analogue scale (VAS), and the patients' life activities were assessed with the Western Ontario rotator cuff (WORC) index. Results: The VAS of the two groups showed improvement at 1 week (aprotinin group: 2.9±0.7, triamcinolone group: 3.7±1.2) and 4 weeks (aprotinin group: 2.1±1.0, triamcinolone group: 2.4±1.0) after injection compared with pre- injection status (aprotinin group: 8.6±1.3, triamcinolone group: 8.2±1.3)(p<0.01) and the WORC index of the two groups showed improvement at 1 week (aprotinin group: 36.5±7.8, triamcinolone group: 53.2±12.3) and 4 weeks (aprotinin group: 33.4±6.2, triamcinolone group: 31.4±8.8) after injection compared with pre-injection status (aprotinin group: 116.2±29.1, triamcinolone group: 123.5±37.0)(p< 0.01). There was no significant difference in the improvement of the VAS scores and WORC index between the two groups. Conclusion: The short term effect of paratendinous aprotinin injection in patients with shoulder tendinitis was as good as triamcinolone injection, although more frequent injection was necessary. The Usefulness of Ultrasonographic Evaluation in the Tendinitis.

Objective
The purpose of this study is to investigate the usefulness of ultrasonographic evaluation in the tendinitis.

Method: 52 tendinitis cases were evaluated by physical examination, Cyriax selective tension technique, simple X-ray and the high-resolution realtime ultrasonography. And clinical diagnosis of tendinitis was confirmed when symptom was relieved after the local lidocain injection to suspected site of tendinitis. The ultasonographic results were compared to clinical diagnosis and ultrasonographic findings were investigated.

Results: The distribution of clinical tendinitis was as follows; 15 extensor carpi radialis tendinitis, 12 supraspinatus tendinitis, 9 subscapular tendinitis and etc. Clinical diagnosis was matched with ultrasonographic results in 38 cases out of 52 (73.1%). The ultrasonographic findings of tendinitis were as follows; 31 hypoechogenicity, 14 thickening, 13 hypervascularity, 12 irregular margin and etc.

Conclusion: The ultrasonography is useful in diagnosis of tendinitis and can provide the exact lesion site of tendinitis. Also it can be used for differential diagnosis of acute or chronic tendinitis. (J Korean Acad Rehab Med 2003; 27: 245-249)

The Usefulness of Ultrasonographic Evaluation in the Musculoskeletal Disease.

Objective: The aim of this study is to know the usefulness of ultrasonographic evaluation in the musculoskeletal disease.

Method: Thirty-nine cases with musculoskeletal pain were evaluated by physical examination, Cyriax selective tension technique, simple X-ray, electromyography, arthrography, Computed tomography, Magnetic resonance image and the high-resolution realtime ultrasonography to define the location of pain. The ultasonographic results were compared to other diagnostic methods.

Results: The ultrasonographic findings are as follow; 22 tendinitis, 7 entrapment neuropathies, 6 bursitis, 2 ligament injuries, and 2 rotator cuff injuries. Clinical diagnosis were matched with ultrasonographic findings in 30 cases out of 39. The most common ultrasonographic findings in 22 tendinitis cases were hypoechogenicity in affected tendon. Ultrasonographic findings in 7 entrapment neuropathies were 6 nerve swellings and 1 nerve flattening.

Conclusion: The ultrasonography can provide detailed images of musculoskeletal system, including tendons, nerves and subcutaneous tissue. Therefore the ultrasonography is very useful in diagnosis and treatment of some musculoskeletal diseases such as tendinitis and entrapment neuropathy.
