Effect of Type D Personality on Short-Term Cardiac Rehabilitation in Patients With Coronary Artery Disease

Objective
To investigate the effect of type D personality on cardiac rehabilitation (CR) participation rates and the effect of a short-term CR program.
Methods
Study participants included patients diagnosed with acute coronary syndrome who underwent percutaneous coronary intervention. Patients completed the Type D personality Scale (DS-14) and the Hospital Anxiety and Depression Scale (HADS) at program entry. Subjects were recommended participation in 6 weeks of CR exercise training. Cardiopulmonary exercise test (CPET) was conducted before and after completion of the training. CR participation refers to completion of the 6-week CR exercise program and performance of the secondary CPET. Drop-out refers to the subjects who were unable to participate in the 6-week CR exercise program or to perform the secondary CPET.
Results
At baseline, type D personality was evident in 21 of 63 patients (33.3%). Type D patients were more often depressed (57.1%) and anxious (38.1%) than non-type D patients (31.0% and 9.5%, respectively). At baseline, participants with type D personality showed a decreased body mass index (24.6 vs. 26.1 kg/m2, p=0.025). The type D group displayed a lower CR participation rate (5/21, 23.8%) compared with the non-type D group of (22/42, 52.4%). Logistic regression analysis revealed the association of type D personality with CR drop-out rate (odds ratio=3.87; 95% confidence interval, 1.2–12.5; p<0.05).
Conclusion
Type D personality was independently associated with drop-out from CR program and with significantly higher levels of anxiety and depressive mood.

Correlations between Pre-morbid Personality and Depression Scales in Stroke Patients

Objective

To investigate the correlation between pre-morbid personality and depression scales in patients with stroke.

Method

The subjects of this study included 45 patients with stroke and their caregivers. We conducted an interview of patients with Beck Depression Inventory (BDI) and also evaluated general characteristic (age, sex, location of lesion, cause of stroke, duration of illness, educational background, history of medication for depression) and functional level. Caregivers were evaluated with Hamilton Rating Scale for Depression (HRSD) for depressive mood, with NEO-PI (Neuroticism, Extraversion and Openness Personality Inventory) for pre-morbid personality. The results of each questionnaire were analyzed in order to investigate their correlation. The results were statistically analyzed with independent t-test, ANOVA, and Pearson correlation test.

Results

The HRSD score of the caregivers had a significant correlation with the BDI score (p=0.001) of the patients. The BDI score correlated with Neuroticism (p=0.021) and the HRSD score also correlated with Neuroticism (p=0.015). There were no statistical correlation of depression with sex, age, case of stroke, location of lesion, duration of illness and functional level.

Conclusion

Among pre-morbid personalities, neuroticism of NEO-PI is the only factor which is significantly correlated with depression scales in stroke patients. Evaluating pre-morbid personality can be helpful in predicting the depressive mood in stroke patients, so we may have early intervention for it.

Psychological Characteristics of the Patients with Myofascial Pain Syndrome in Shipyard Work.

Objective
This study was conducted to analyze the psychological factors which influence myofascial pain syndrome and to evaluate whether the electromyographic study on the myofascial trigger point in shipyard workers satisfied the diagnostic criteria of myofascial pain syndrome. Method: We studied 61 patients who were employed in the shipbuilding industry and diagnosed myofascial pain syndrome. We investigated them using the Minnesota Multiphasic Personality Inventory (MMPI) and the visual analogue scale (VAS) and also obtained spontaneous electrical activity (SEA) by electromyographic study on myofascial trigger point.Results: Mean VAS of subjects was 5.88. On the MMPIprofile, the patients scored highest in psychopathic deviate, and then in hypochondriasis, depression and hysteria, in that order. The correlation of the VAS with results of electromyographic study and with the MMPI were not significant. In the electromyographic study, 19 patients out of 61 (31%) showed initial positive deflection on spontaneous electrical activities. Conclusion: This study suggested that considering the psychosocial aspect of myofascial pain syndrome, as well as organic aspects in hard manual labor such as shipyard work might be useful for treatment. Psychological Investigation in Myofascial Pain Syndrome Patients.

The myofascial pain syndrome patients usually have characteristic personalities. The purpose of this study is to investigate the psychological characteristics of myofascial pain syndrome patients. We reviewed medical records of fifty-four patients with myofascial pain syndrome for their Minnesota Multiphasic Personality Inventory, Visual Analogue Scale, and psychological interview notes.

On MMPI profile, the patients scored high in hypochondriasis, hysteria and depression scales, which are the triad of neurosis. According to stress factors and occupations, the psychologic responses were different. We have compared the severity of pain according to different life stress and occupations. VAS scores of the student group were higher statistically in the occupation comparison and of the marital problem group were higher in the life stress comparison.

It is suggested that psychologic intervention on myofascial pain syndrome patients should be stressed for the management of pain.
