Physical Activity and Activities of Daily Living in Older Adult Patients With Heart Failure Admitted for Subacute Musculoskeletal Disease

Objective
To examine activities of daily living (ADL) and physical activity in older adults with heart failure admitted to a rehabilitation ward for subacute musculoskeletal disease.
Methods
This study included patients with musculoskeletal disease (aged ≥75 years) who were admitted to the rehabilitation ward. Data on age, ADL, and time for physical activity (metabolic equivalents [METs]) were collected. Patients were divided into groups with or without heart failure, and the differences were compared using Mann–Whitney U-test.
Results
This study included 84 musculoskeletal patients, including 25 with heart failure. The heart-failure group had similar levels of ADL independence compared to the without-heart-failure group (p=0.28) but had shorter duration of continuous and sustained physical activities and less total time (p<0.01) of light-intensity physical activity or higher.
Conclusion
Older adults with subacute musculoskeletal disease with heart failure do not necessarily require a large amount of physical activity to maintain ADL at the time of discharge. But very low physical activity may increase the risk for developing hospitalization-associated disability. Physical activity in older adults with subacute musculoskeletal disease with heart failure should be monitored separately from ADL.

The Prevalence and Characteristics of Depression in Work-related Musculoskeletal Disease

Objective

To reveal the relationship between depression and WMSD.

Method

Five physiatrists participated in the workplace musculoskeletal survey and diagnosed 724 office workers with WMSD by performing detailed history taking and physical examination. All subjects were asked to answer the Korean version of the Beck depressive inventory (K-BDI), and to express their pain according to the visual analogue scale (VAS) score. We categorized the subjects into 4 groups, myofascial pain syndrome (MPS), herniated intervertebral disk (HIVD), tenosynovitis, and others, and investigated the prevalence of depression in desk workers and relationship between WMSD and depression, and we compared pain intensity between the depression and non-depression groups. Correlation analysis was carried out between K-BDI and VAS scores in each group.

Results

The mean K-BDI score were 8.7±6.68. The prevalence of depression was higher in females than in male, and there was no relationship between age and depression. There was a significant connection between HIVD and depression (p<0.05). However, the other groups did not have significant connection to depression. The VAS score (5.02) of the depression group was significantly higher than that (4.10) of the non-depression group. In addition, there was a significant difference of VAS scores between the depression group and non-depression group in each disease group.

Conclusion

The mean VAS score of the depression group in WMSD was significantly higher than in the non-depression group. The correlation between BDI and VAS scores in the subjects was present, and the highest was in the HIVD group.

Disability Duration in Musculoskeletal Injured Patients due to Automobile Accidents.

Objective
The aims of this study were to refer descriptive statistics of the disability duration in musculoskeletal injured patients in the automobile accidents. Method: The authors chose 469,319 injured patients in traffic accidents as subjects that met inclusion criteria with 8 representative Abbreviated Injury Scale (AIS) comparable with 4th edition of Korean standard classification of diseases. Mean, standard deviation, mode, median, maximum and minimum of 8 representative AIS code were calculated. Results: Mean disability duration for minor cervical sprain was 18.8 days, that of moderate cervical disc herniation was 56.2 days, that of serious cervical disc herniation was 141.5 days, that of minor lumbar sprain was 21.2 days, that of moderate lumbar disc herniation was 53.5 days, that of serious lumbar disc herniation was 128.1 days, that of sprain of 3 major joint in lower limb without desmorrhexis was 24.0 days, and that of dislocation or desmorrhexis of 3 major joint in lower limb was 101.2 days. Conclusion: Disability duration of the representative musculoskeletal diseases comparable to AIS code in 4th edition of Korean standard classification of diseases was presented and this study can be utilized as a basic recommended disability duration of automobile injured persons. Characteristics of Disability Recuperation Musculoskeletal Diseases at a Ship-Yard.

Objective
This study was performed to analyze the length of work-related musculoskeletal disability and to identify the various related factors of disability duration. Method: Approved cases work-related musculoskeletal disease between Jan 1, 2000 and Jan 6, 2004 were collected by using the workers' compensation company's database and questionnaire. The cases were comprised of 16 more frequent musculoskeletal diseases. 551 approved musculoskeletal disease cases were composed of 295 recuperated cases and 256 recuperating cases. Results: The average length of work-related musculoskele- tal disability were 420.3⁑246.2 day in the recuperated group and 428.0⁑328.9 day in the recuperating group. The mean disability length of musculoskeletal diseases were over one year with similar distribution except for ganglion. The length of disability were not affected by age, employment duration, type and number of co-mobidity, time from claim receipt to allowance, type of treatment. But, operation history and accident type were affected to the length of disability. Conclusion: The results suggest that the length of work- related musculoskeletal disability was not determined by biomedical disease related factors, however, it determined by social and social insurance systemic factors. The Usefulness of Ultrasonographic Evaluation in the Musculoskeletal Disease.

Objective: The aim of this study is to know the usefulness of ultrasonographic evaluation in the musculoskeletal disease.

Method: Thirty-nine cases with musculoskeletal pain were evaluated by physical examination, Cyriax selective tension technique, simple X-ray, electromyography, arthrography, Computed tomography, Magnetic resonance image and the high-resolution realtime ultrasonography to define the location of pain. The ultasonographic results were compared to other diagnostic methods.

Results: The ultrasonographic findings are as follow; 22 tendinitis, 7 entrapment neuropathies, 6 bursitis, 2 ligament injuries, and 2 rotator cuff injuries. Clinical diagnosis were matched with ultrasonographic findings in 30 cases out of 39. The most common ultrasonographic findings in 22 tendinitis cases were hypoechogenicity in affected tendon. Ultrasonographic findings in 7 entrapment neuropathies were 6 nerve swellings and 1 nerve flattening.

Conclusion: The ultrasonography can provide detailed images of musculoskeletal system, including tendons, nerves and subcutaneous tissue. Therefore the ultrasonography is very useful in diagnosis and treatment of some musculoskeletal diseases such as tendinitis and entrapment neuropathy.
