1978년말 경제계획원보고에 따르면, 우리나라 인구당 GNP는 1242불, 년간성장률 15.5%로 후진국을 완전히 벗어나 발전국으로서의 거보를 다짐하고 있다. 이와 같은 경제발전에 따라 보건문제에도 변모를 볼 수 있어, 질병예방 및 환경위생의 향상 및 의료시설의 확충등으로, 사망률은 저하하고 평균수명은 높아지고 있어 우리나라의 국민보건 양상에 큰 변화를 가져오고 있다. 질병으로 인한 사망률의 저하는, 교통사고 및 공장사고 등으로 인한 신체기능장애자의 증가를 초래하고 있으며, 또한 만성질환자의 수도 많아져 이들에 대한 치료수단이 감구되어야 할 입장에 있다. 재활의학은, 이들 환자들의 사회복귀의 필연적 치료과정의 하나이며, 재활의학에 대한 의학교육을 의과대학생에게는 물론, 졸업후 전공의 교육으로서도 고려되어야 할 문제이다. 본 고려대학에서는 이 문제에 관심을 가지고 1972년부터 재활의학의 전공의 수련제도를 실시하고 있으므로 이 수련계획을 보고하여, 멀지않아 같은 계획이 다른 교육병원에서도 실시될 것을 기대하며, 참고가 되리라고 믿고 그 교육과정 일절을 소개한 것이다.
Every human being carries some defective genes and there is no way of making sure that his or her parther is not accidentally heterozygous for the same allele. Therefore, the genetic counselor must make every effort to banish all false guilt or responsibility considerations from the discussion It must be made absolutely clear that the purpose is simply to prevent the misery imposed on the individual as well as his family through the birth of defective children.
In this paper, to prevent the birth defect and mental redadation, the methods of antenatal diagnosis by techniques for antenal diagnosis and recurrence risk figures(probability), and mode of genetic inheritance are described.
The pain of the ankle and foot was encountered in a large group of patients whose disabilities arise in association with weight bearing stress on the foot and the affections of the heel include the results of inflammation and injury. The pain around the ankle occurs at the site of insertion of Achilles tendon and subtalar joints.
A clinical review and analysis was carried out on 170 cases of 154 patients in ankle and foot pain who visited O.P.D of Orthopaedic Surgery, Catholic Medical College during 8 months of the period from March 1978 to October 1978. Most cases were treated conservatively and various method of treatment were adapted according to the condition of the disease.
Results of this study are as follows;
1. There was no definite differences between the sex distribution, but the incidence in females was slightly greater than in males in disease group, the incidence in males was slightly greater than in females in trauma group.
2. 48% of the 154 patients was found to occur between the age of 10 and 29 years.
3. Difference between the incidences of the right and of left foot was not determined.
4. Most of the cases involved the hind foot involving ankle joint (2/3 of all cases).
5. In disease group the rheumatoid arthritis involved 22 cases among 89 cases, and the ankle sprain and the fracture involved 61 cases among 81 cases.
6. The average duration from onset to visit O.P.D. within 4 weeks was 34.7% in disease group and 63.3% in trauma group.
7. Tenderness and swelling was noticed in about 50% in physical examinations and the abnormal finding was not found in 22.4% in X-ray examinatons.
8. Most cases was treated with a medication combind with rest and external support.
Carpal tunnel syndrome is frequently accompanied or preceded by certain lesion such as trigger finger or thumb, diabetes mellitus, rheumatoid arthritis, periarthritis-shoulder, etc. It is rather rare and of some interest to observe CTS with dermatomyositis. Recently such a case was seen at SNUH and reported hereby. The electrodiagnostic finding was characteristic of dermatomyositis (or polymyositis) and concomittant CTS:
1. Myopathic pattern, throughout--i.e., short duration motor unit action potential, short duration polyphasic potential, etc.
2. Abundant abnormal spontaneous activity--Fibrillation potentials, positive sharp waves, high frequency bizzare discharge, insertional activity, more marked in proximal group of muscles.
3. Prolonged latency of motor and absent sensory nerve action potential of Rt. median nerve.
4. Normal nerve conduction velocity.
It is obvious that the essence of rehabilitation of the spinal quadriplegic and paraplegic must lie in restoring the activities of body and mind to re-integrate himself into the community. Thus, the social adjustment is the final achievement and the importance of rehabilitation in "The spinal man".
The social situation of spinal man in our country is quite different from that of the advanced country, since there is no legislation or acts were established upon those of civilian disabled population after they have been discharged from the hospital care.
This study is to observe the encountered social problems and soical adjustment states of 10 spinal cord injured males, ranging ages from 20 to 43 (2 Quadriplegics and 8 Paraplegics), who responded to our follow up inventory study coducted by quetionnaire paper and visiting. The results were analized and their problems and further development procedures are discussed for their social rehabilitation program.
